Your first appointment usually consists of a comprehensive exam and a review of medical history. Treatment can usually be rendered on the same day if not contraindicated by complex medical history.

Please provide us with the following information at the time of your appointment and/or consultation:

  • Any X-rays taken by previous dental provider, if applicable
  • List of medications you are presently taking
  • If you have dental insurances, please bring the necessary insurance information. This will save time and allow us to help you process any claims.

A parent or guardian must accompany patients under the age of 18. Please notify our staff if you have a medical condition that may be of concern prior to treatment (i.e. diabetes, high blood pressure, artificial heart valves, complete joints replacement, fever, etc.) or if you are taking any medication (ie: heart medications, aspirin, anticoagulant therapy, osteoporosis medications, etc.)

For your convenience we accept Visa, Amex, MasterCard, and Discover. We try to deliver the highest quality of care, therefore payment is due at the time service is rendered unless other arrangements have been made in advance. Please don’t hesitate to contact us if you have any questions.

Most insurance companies will respond within 4 to 6 weeks. We will send you a monthly statement if any remaining balances are left after your insurance has paid. Monthly payment plans are available but must be done prior to service rendered.
